Twenty high school students enroll in a competitive course to get ready for a college exam. They include the disobedient Kang I-na, Yoon Myong-heo, I-na's best friend, and Kang Hyun, the class clown who has feelings for I-na. Following incidents involving a student, Beom has a ghostly presence haunting his paper, and classmate Hwang Chang-wook finds scars on the kid's hand.

The lesson is interrupted by a television displaying Min Hye-yeong, the top student, confined inside a glass tank that is gradually filling with water. They are taking part in a death game, which has questions that the class must answer before the student dies, according to a voice in the PA system. Additionally, it forbids them from leaving the school together with their two teachers, Hwang Chang-Wook and Choi So-Yeong.

The class is requested to solve the equation on the glass tank in which their classmate is locked, but they are unable to do so in time to save Hye-yeong, who drowns as a result.

Despite the warning, the class decides to leave, only to discover that Mr. Kim, a security guard, is the only one left to watch over them after Mr. Lee, a hall monitor, was beaten to death. The pupils break off and work on the questions individually while looking for a way out.
One of their classmates had a wax drip on him slowly, and they failed to save him as the wax eventually strangled him in one of the horrifying death games set up by a strange woman as the class solved all the answers too late.
They later realize that each puzzle holds a clue about Kim Ji-won, a student who passed away earlier that year under unexplained circumstances, even though it was reported to be a suicide. In a learns that pupils are passing away based on how they performed on the most recent test.

Beom, who attempted to strangle I-na before and was detained in a mental institution, escapes and tries to kill I-na, but Hyun protects her and is killed in the process. Then, Chang-wook murders Beom. I-na learns to her dismay that the woman has also abducted Myong-her.

The killer nevertheless drops the victim from the roof, and Chang-wook scales the roof to strangle the victim to death, despite their success in escaping the trap.

In the school's gymnasium, a memorial service is held for the kids who died, and then I-na is abducted without anyone's knowledge. One last question is suddenly asked over the PA system: "Whoever killed his daughter must come forward and confess." The perpetrator, who is revealed to be Kim the school security guard, kills the individual in question, the school counselor, and a now-maddening Chang-Wook. So-Yeong then sets I-na free.

Flashbacks show that Kim Ji-won put a lot of effort into getting into the top class. The parents of the murdered pupils, however, bought Chang-wook's cooperation in exchange for the entrance exam solutions, giving them an unfair advantage.
Ji-won confronted Chang-wook after learning about the conspiracy and being disqualified; as a result, he kills her to hide his wrongdoing. Since Ji-won's passing, her parents' lives have crumbled as a result of the family home being taken by loan sharks.
He killed the loan sharks out of rage and then plotted his and his wife's retaliation. Beom, the only witness to the incident, gradually grew insane because he believed Ji-won had been following him ever since.
After learning this information via their daughter's cellphone, which she had been using to record herself being strangled, Kim and his wife—the person who hanged Myong-he—decided to kill Chang-wook and all the parents of the students whose parents they believed were to blame for their daughter's death.

The movie ends with I-na learning that she came in first place in her class, and it is hinted that Beom's attacks were caused by the fact that she had been Ji-won's possession from the start because she is briefly depicted as Ji-won.
